Low Back Ability vs Low Back Pain Coach

Written for adults with long-running low back pain who are cleared to exercise. I have no medical training; this is the method I used and what the research says about it. Some symptoms need a doctor before any exercise: check the red flags.

Low Back Ability is a membership built around a coach's roadmap of exercise videos and a group chat for members, and you choose what to pay. Low Back Pain Coach is an app that sets each back-extension session from your pain log, with floor work until you own a bench and a fixed price.

I built Low Back Pain Coach and make money when people buy it. Low Back Ability (LBA) is sold by Low Back Ability LLC, and its founder is Brendan Backstrom. For LBA I used only its website, App Store listing and YouTube channel as they stood on September 25, 2026. All are linked under Sources.

What Low Back Ability offers

LBA's program page lists a roadmap of form videos with sets, reps and technique for each exercise, which it says you adjust to your level, plus a group chat for members and lessons on anatomy, flare-ups, tissue tolerance and timeframes for healing. Its FAQ says the roadmap is for members only. The YouTube channel is free and carries long training videos and interviews posted as the Long Game Podcast.

Training runs as two sessions, the Flow (11 movements) and the Mobility Flow (9 movements), the same for every member and picked from a calendar in the Members Hub. LBA's blog lists its levels as isometric holds, single-leg holds, controlled hinge reps, full-range reps and loaded reps, and says each level advances only when next-day symptoms stay stable. You set the dose, range and speed yourself from how you respond the next day. The FAQ says there is no true alternative to back extensions.

The iPhone and iPad app came out in July 2026. Its App Store listing calls it the official training app for LBA members, with workout plans, a video for each exercise and progress tracking, and updates since launch added community chat. I found no Google Play listing, and the members area also runs in a web browser.

As of September 2026, the website lets you pay what you can. The monthly subscription has a slider from $3 to $250, a year costs $99, and the price page says every member gets the same experience whatever they pay. The App Store lists pay-what-you-want purchases from $5 to $20 and an annual option. LBA describes itself as an educational training plan, not medical treatment.

What Low Back Pain Coach does

It runs one program, four stages of back extensions on a 45-degree bench. Holds at the top come first, then partial reps, full reps at body weight, and reps with a plate held at the chest. Until you have a bench, floor work (a prone hold, bird-dogs and glute bridges) covers the gap. The program is not written for a vertical 90-degree Roman chair.

You set a baseline, your usual pain on an ordinary day, and log pain after training, normally the next morning. The app reads that log against your baseline. Above baseline, the session is eased. At baseline, it repeats. Below baseline, it steps up a little. Pain logged in the legs stops the extension work. A spoken voice counts holds, reps and rest. Flare-up mode pauses bench work and restarts it at an easier dose after two days at or below baseline, and its screen asks about red-flag symptoms first. A PDF report sums up your logs and sessions for a clinician.

It is a free download with one free guided session. After that it costs $9.99 a month or $59.99 a year, renewing until you cancel, each with a 7-day free trial for eligible new subscribers, or $149.99 once. It runs on iPhone and Android. The app is not a treatment, and this exact progression has not been tested in a trial.

Who each might suit

Low Back Ability might suit you if you want a coach's video library and a chat full of people doing the same work, and you would rather judge each day's dose yourself at a price you pick.

Low Back Pain Coach might suit you if you already own a 45-degree bench and want each session set from the log you made the morning after the last one, with a voice counting so the phone can stay on the floor. Without a bench yet, the app starts you on floor work, and the bench buying guide covers what to look for. The app page shows how each session gets written.

What they share

Both center on the back extension, moved forward in small steps from holds toward added weight, and both use how you feel the following day to decide the next step. The progression page lays out my four stages, and how it works explains why the program is built on a bench.

Neither one examines you. New, severe or changing symptoms need a clinician, and the red flags page lists the ones that cannot wait.

If you use both, let one of them set the back-extension dose so two plans do not stack load on the same days. LBA's FAQ also asks members to limit other major training aimed at the hips and low back.

The app

I built Low Back Pain Coach to run this program: it reads your pain logs, writes the next session, and a voice counts the holds, reps and rest. A notebook can run the same rules if you would rather keep the decisions on paper.

If you want an email when the program or the app changes, leave your address. A few emails a year at most, and a reply to any of them takes you off the list.
