Is Yoga Good for Lower Back Pain? Cochrane and Guidelines

Short answer

Wieland and colleagues' 2022 Cochrane review (21 trials, 2223 participants) found low- to moderate-certainty evidence that yoga, compared with no exercise, results in small and clinically unimportant improvements in back-related function and pain in adults with chronic non-specific low back pain. At three months, function was 1.69 points lower (better) on a 0 to 24 scale (mean difference -1.69; low certainty; 11 trials, 1155 participants) and pain was 4.53 points lower (better) on a 0 to 100 scale (mean difference -4.53; moderate certainty; nine trials, 946 participants), below the review's clinically important differences of 5 and 15 points. Compared with other back-focused exercise, there is probably little or no difference in function at three months, and any difference in pain is uncertain. Yoga is associated with more adverse events than no exercise, primarily increased back pain, but may have the same risk as other exercise.

I have no medical training. This page reports Wieland's 2022 Cochrane review of yoga for chronic non-specific low back pain, NICE NG59 on exercise for low back pain with or without sciatica, and the December 2023 WHO news release on chronic primary low back pain. It is not a test of the bench program in the app. Some symptoms need emergency care or a clinician before any exercise: check the red flags.

Cochrane's 2022 review of yoga for chronic non-specific low back pain, by Wieland and colleagues (21 trials, 2223 participants), found small improvements in back-related function and pain compared with no exercise, and calls them clinically unimportant. The certainty was low to moderate. Compared with other back-focused exercise, there is probably little or no difference in function at three months, and any difference in pain is uncertain.

Is yoga better than other exercise for back pain?

Whether there is any difference for pain and quality of life remains uncertain.

Do guidelines recommend yoga for chronic low back pain?

NICE NG59 recommendation 1.2.2, marked 2016, says to consider a group exercise programme, including mind-body approaches, for a specific episode or flare-up of low back pain with or without sciatica, and does not specifically recommend yoga. WHO's 2023 release recommends exercise programs for adults with chronic primary low back pain and does not specifically recommend yoga.

Can yoga make back pain worse?

In Wieland's 2022 review of adults with chronic non-specific low back pain, low-certainty evidence from eight trials found more adverse events with yoga than with no exercise, primarily increased back pain (43 per 1000 with yoga, 9 per 1000 with no exercise, at six to 12 months). Yoga may have the same risk of adverse events as other exercise.

When should I stop and see a doctor?

Go to emergency care now for any of these: New numbness, tingling or change in feeling in the saddle area: around the groin, inner thighs, genitals or anus, or when you wipe; New trouble getting or keeping an erection, not being able to orgasm, or a change in feeling during sex; New bladder or bowel changes: trouble starting or stopping pee, a weak stream, not feeling when you need to go, or leaking pee or poo; New pain, tingling, numbness or weakness in both legs; Leg or foot weakness that is getting worse (tripping, a foot that slaps down); Back pain that started after a recent serious accident, like a car crash or a bad fall. See a clinician today (urgent care if no appointment today) for any of these: Other new weakness or numbness in one leg or foot; Fever or chills alongside the back pain; New pain after a recent smaller fall or knock; Unexplained weight loss, or a history of cancer; Pain that is rapidly getting worse; Pain that is constant at rest and worse at night, not tied to movement.
